bfdd: Standardize prefix output, %pFX, in debugs

Currently there are two problems with using %pFX in bfdd
    debugs:

    a) The vrf name, vrf id and table were not being output for
every debug. When a prefix is used in multiple vrf's it
is difficult to know if the debug being looked at is for
    the instance of interest.

    b) The output was not standardized. Solidify on
    "(%s:%u:%u)%pFX", vrf_name, vrf_id, table_id, prefix. This
    allows very easy grepping.

The only %pFX debug is the zebra interface address add/delete
event, which previously printed the vrf id as a trailing
"(VRF %u)". Use the interface's vrf for the name and table id.
