    Build/Test Tools: Keep default-excluded groups excluded when using `--exclude-group`.

    Passing `--exclude-group` replaces the group exclusions in the PHPUnit XML config, and the test bootstrap also cleared its own list of skipped groups. So any `--exclude-group` run also ran the groups excluded by default, such as `ajax`, `ms-files` and `external-http`.

    A new test listener now removes default-excluded tests from every suite, including nested ones, before class fixtures run. A group requested with `--group` still runs.
